Brief Description of Duties:
JOB SUMMARY: The Project Management Coordinator will plan, organize, and coordinate special projects and ensure project initiatives and timelines are met. The incumbent will partner with management and project teams to design project plans requiring assessment, research, and analysis, as well as coordinate implementation of project plans and evaluate outcomes. This position has a unique opportunity to improve clinical fundamentals across the continuum of care including outcomes in quality, access, cost, and patient and staff satisfaction. In addition, the incumbent will contribute to ensuring DSRIP goals and objectives are met. DUTIES INCLUDE: Lead and organize multi-disciplinary teams in developing a new model of care that reduces preventable hospital admissions and readmissions; provides comprehensive, compassionate, and patient-centered care; and includes preventive care, care coordination for the chronically ill, and continuity of care. Understand, analyze, and translate the impact of DSRIP projects and initiatives and how these projects and initiatives will fundamentally change the healthcare delivery system. Identify barriers to project implementation and escalate any risks to PMO and DSRIP V.P. Utilize best practices and OneCity Health Project Plan, toolkits, model of care and all resources to deliver on project plans and implementation strategies. Represent UHB in a positive manner by demonstrating respect and courtesy to all internal and external customers. Practice the Principles of Service Excellence in all interactions with patients, families, and colleagues. Create a workgroup environment that is focused on cultural competence and health literacy in relation to the population served. Communicate effectively with Project Director, Executive Director and upper management, One City Health, CBO’s and multi-disciplinary team. Ensure adequate resources are allocated to each work team and facilitate project implementation. Ensure multi-disciplinary representation on each work group Identify work team member performance issues and escalate as appropriate Understand the Process and quality improvement measures Assist in analyzing and auditing data (reports) to ensure accurate reporting to OneCity Health and State Department of Health but also to monitor implementation and PI/QI process Monitor progress of each newly implementation and risk and barriers that will prevent a successful implementation. Project Coordinator would measure success of each of the project implementation through data analysis and reporting and ensure the integrity of all reported data All other tasks related to the transformation initiatives and their sustainability.

|All successful candidates must undergo various background checks, maintain credentials required for continued employment and adhere to the SUNY-DMC UHB Principles of Behavior. Clinical Faculty and Allied Health professionals must receive and maintain Medical Board authorization.|
REQUIRED: RN/LMSW/Certified Project Manager, PMP. 3-5 years primary care clinical experience. Leadership experience working with multidisciplinary teams. In-depth understanding of change management principles. Skill working in matrix reporting relationships where success is achieved through joint efforts, teamwork, multi-disciplinary approaches, influence vs authority, and collaboration. PREFERRED: Master's degree. Familiarity with Smartsheet software's collaboration and time management tools.
