The Pharmacy Department at SUNY Downstate is seeking a full time Pediatric Senior Clinical Pharmacist. The Senior Clinical Pharmacist, Pediatrics will provide clinical pharmacy services to pediatric and neonatal populations at the State University of New York (SUNY) Downstate Health Sciences University. The Senior Clinical Pharmacist will work as part of a multidisciplinary team to ensure the delivery of safe, evidence-based, and cost-effective drug therapy. The successful candidate will:
- Participate as a vital clinical team member during interdisciplinary rounds to promote safe and cost-effective pharmaceutical care in state-of-the-art Pediatric and Neonatal Intensive Care Units.
- Serve as the primary drug information resource and pharmacist liaison for the pediatric and neonatology services.
- Monitor pharmacotherapy and recommend adjustments to medication regimens and nutrition support by utilizing appropriate pharmacokinetic/pharmacodynamics principles, evidence-based medicine, and patient-specific factors.
- Detect and report medication safety events and adverse drug reactions. Participate in medication safety initiatives within the pediatric patient population and within the hospital.
- Obtain accurate medication histories as needed, confirm appropriate and updated documentation in the electronic medical record, and ensure accurate medication reconciliation has been performed during inpatient admissions.
- Provide discharge counseling as needed to patients and/or caregivers on medication therapy in conjunction with physicians and nurses.
- Serve as a member of the Pharmacy & Therapeutics Committee, internal pharmacy-specific committees, and pediatric-specific performance improvement committees (e.g., Pediatric Performance Improvement Committee, Neonatal Performance Improvement Committee, Pediatric Critical Care Committee, etc).
- Design and deliver educational programs on pharmacotherapy to pharmacy, medical, and nursing staff.
- Precept and mentor pharmacy residents and pharmacy students from local colleges of pharmacy.
- Participate in scholarly activities including peer-reviewed publications, medication use evaluations, clinical research projects, and cost-savings initiatives.

DUTIES AND RESPONSIBILITIES ARE NOT LIMITED TO THE ABOVE POSITION DESCRIPTION

All successful candidates must undergo various background checks, maintain credentials required for continued employment and adhere to the SUNY-DMC UHB Principles of Behavior.   Clinical Faculty and Allied Health professionals must receive and maintain Medical Board authorization.
Qualifications:
REQUIRED:
- Doctor of Pharmacy (PharmD) from an ACPE-Accredited College of Pharmacy.
- New York State Pharmacist Licensure or Eligibility.
- Completion of a PGY1 Pharmacy Residency.

PREFERRED:
- Completion of a PGY2 Pediatric Pharmacy Residency OR Completion of a PGY1 Pharmacy Residency in a pediatric hospital setting with 1+ years of independent practitioner experience OR 3-5 years of equivalent pediatric experience.
- Board Certified Pediatric Pharmacotherapy Specialist (BCPPS).
